At approximately 1:40 PM, Licensing Program Analyst (LPA) Contreras and Licensing Program Manager( LPM) Mota arrived unannounced to conduct a Case Management visit, and met with caregiver Ma Gracia Manalo. The purpose of the visit was to obtain further documents regarding a change of administrator and documents requested at the annual inspection completed by LPA Canela on 3/7/2025. The following documents are required for the change of administrator and prior annual inspection: LIC500 Personnel Report LIC308 Designation of Facility Responsibility Copy of current Liability Insurance Copy of current Lease Agreement Last page of LIC610 Disaster Plan if there are no changes Licensee was unavailable at today's visit but was contacted by telephone and informed LPM that a copy of the above documents will be emailed.
