What the inspector wrote

Licensing Program Analyst (LPA) Alviso conducted a Required- 1 Year inspection, at about 10:30am on 3/17/26, and met with Licensee Suzette Tawser. Currently there are five (5) residents in care; Two (2) residents are on hospice care. Facility is fire cleared for six (6) bedridden residents; All resident rooms may be occupied by bedridden, non-ambulatory and/or ambulatory residents. Facility is approved for six (6) hospice residents' Facility has a required emergency disaster plan. Facility has a required infection control plan. Facility has a dementia plan of operation. LPA reviewed four five (5) resident files. All files were complete. LPA reviewed three (3) staff files. All staff have criminal record clearance. All staff have first aid certification and CPR certification. LPA reviewed staff training. LPA toured the facility with the Licensee. Hot water was measured at 114.4 degrees Fahrenheit. All exits were free and clear of obstruction. Fire extinguishers were serviced and tagged. LPA observed the food supply to be sufficient. Facility was at a comfortable temperature during the inspection. Facility bathrooms had grab bars, and non-slip mats for residents' use. Facility had all medications locked and inaccessible to residents' in care. Facility had cleaners/disinfectants locked up and inaccessible to residents' in care. Sufficient supply of hygiene products, linens, furnishings, personal protective equipment (PPE), and paper products. Continued on LIC809C.. Backyard had a large wooden patio deck that has fire exit ramps on each side of the home off of the patio deck. Exit gates opened freely during the inspection. Smoke detectors were in all resident rooms and required common areas, and carbon monoxide detector was up, all working during the inspection. LPA is requesting the following documents be updated and submitted by 4/17/26: LIC308 - Designation of Administrator Responsibility LIC500 - Personnel Report LIC610E-Emergency Disaster Plan (ensure to review and update as needed/required-no changes submit copy of review page) If changes submit plan copy Infection Control Plan (ensure to review and update as needed/required- no changes submit copy of review page)- If changes submit plan copy LIC400 Handling of Client Cash Resources (all facilities complete this-include copy of surety bond if handling cash) Current Liability Insurance Resident Roster Copy of current Administrator Certificate No deficiencies cited during today's inspection.
