Licensing Program Analyst Leibert arrived unannounced for the purpose of delivering findings on this complaint. During the course of this investigation statements were taken, documents were obtained and reviewed and site visits were made to the facility. The following determinations are made: Shower logs for the available period for R1 indicate that R1 was given showers in compliance with R1's care plan; Care notes indicate R1 periodically refused staff efforts to provide care, including showers; Complainant claims to have documentation from R1's dentist that would support the allegation that staff do not assist with oral hygiene; Complainant has not produced any documentation regarding R1's oral hygiene; This Department has made many requests for the documentation. Although the allegations may be true, based upon the statements and documents, there is not a preponderance of evidence to prove or, disprove, the allegations. Therefore, the allegations are UNSUBSTANTIATED. Report left.
