Licensing Program Analyst (LPA) Elias Magdaleno arrived unannounced to conduct a case management- incident inspection and met with Morgan Whinery, Administrator. LPA is conducting a case management visit to obtain more information regarding an Incident Report received by the Department on 8/13/2025, involving a resident (R1) being sent out to the hospital and subsequent transfer to a skilled nursing Facility (SNF). LPA obtained copies of R1 care plan and face sheet. No deficiencies were cited during todays visit. Exit interview was conducted with Administrator, whose signature on form confirms receipt of document.
