At approximately 10:15AM, Licensing Program Analyst (LPA) Chris Arnhold arrived at this facility unannounced, to conduct a case management visit in regards to an incident report submitted by the facility on 06/02/2025. LPA met with Assisted Living Care Coordinator Beatriz Cortez and reviewed records. Based on records reviewed, Resident (R1) contacted law enforcement and threatened to cause physical harm to another resident. Law Enforcement arrived at the facility and was not able to locate R1. According to staff, R1 had left the building. LPA reviewed records and observed R1 is able to leave unassisted, based on a physician report created by a physician who has treated R1 since 2014. R1 returned to the facility and was asked about the telephone call. R1 did not have recollection of making the call. LPA reviewed records and observed facility has been in constant contact with physician and responsible party and the care plan is being updated as needed. No citations issued during today's visit.
