At approximately 2:00PM, Licensing Program Analyst Contreras arrived unannounced to conduct a case management visit for an incident that was self-reported by facility. From 12/25/25-12/30/2025 Resident (R1) was admitted to emergency hospital for vomiting. R1 was under observation in Kaiser Vacaville. R1 was sent home on 12/30/25. On 1/01/2026 R1 returned to the hospital for change of condition again due to refusing to eat and take medications. After visit summary showed R1 was diagnosed with dehydration. R1 had appointment with PCP for observation. R1 has not experienced any other changes of condition since returning back from the hospital. Admin discussed with LPA after visit summary from Hospital and copy retrieved for file. Facility admin has discussed a plan with staff to prevent dehydration from occurring and changes of condition of resident. No deficiencies cited during todays visit.
