What the inspector wrote

At approximately 11:45 AM Licensing Program Analyst Star Stevenson arrived unannounced for a case management visit and was greeted by employee (E1) E1 was observed to be providing food and drink to one resident and helping to guide another confused resident over threshold from the kitchen to sunroom. E1 was noted, to not be associated to the facility on the Guardian roster report run today, 10/21/2025. When E1 was asked if they had a drivers license or social security number to inquire about background clearance, E1 responded that they did not. E1 was also providing care for residents on 09/25/2025. A repeat violation of Personnel Records: CCR 87412(a)(13)(B) was issued in the amount of $200 The repeat deficiency was cited from the California Code of Regulations, Title 22, Division 6 of California Regulation. Failure to correct the deficiency and/or repeat deficiencies within a 12 month period may result in civil penalties. Exit interview conducted with Administrator Charmaine Mendaros Appeal rights provided for the Administrator.
