05/04/2026, Licensing Program Analyst (LPA) Loera conducted an unannounced Annual Required – 1 yr. inspection visit for this facility. There are currently no residents in care. Facility has an approved fire clearance for 6 non-ambulatory. Upon arrival no staff were present. Administrator, Reggie Capili arrived at approximately 10:30am. LPA and Administrator toured the building and grounds. Facility has 4 bedrooms, 2 bathrooms, a living room, and an activity room. The facility was found to be at a comfortable temperature. All rooms were furnished and in good repair. Extra hygiene products and linens were available. Water temperature in sinks were measured and found to be within the range of 105 to 120 degrees F. Fire extinguishers were found to be recently purchased. Smoke/Carbon Monoxide detectors located throughout the facility are hardwired and were tested and operational. Sharps were located in a locked box in a kitchen drawer. Chemicals were found to be inaccessible in the garage. Facility has a medication storage closet in the hallway where medication will be kept. LPA conducted review of two staff records. Upon a review of staff records, LPA found staff to have required training as well as current 1st Aid & CPR certification on file. Administrator certificate for Reggie Capili is current from 01/18/2025 - 10/22/2027. No deficiencies cited during today's inspection. Updated copies of the following documents were requested for facility file and are to be submitted to CCL by 06/04/2026: LIC500- Personnel Report LIC308- Designation of Responsibility Updated Liability Insurance LIC610- Emergency Disaster Plan (review, update if needed) Exit interview conducted with Administrator and a copy of this report was provided.
