Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a Post Licensing Inspection and was greeted by administrator (admin) Clerr Ruiz. Facility is an Adult Residential Facility for the Elderly that has an approved fire clearance approved for capacity of 6 ambulatory and non-ambulatory residents. There are currently 3 residents at facility. LPA and admin toured all buildings and grounds and were found to be clear from obstruction. Facility was found to be at a comfortable temperature. All required postings were visible. There was a sufficient supply of perishable and nonperishable foods as required by Title 22 Regulations with expiration dates noted. LPA observed medication to be pre-poured for one resident. LPA spoke with admin that medication cannot be pre-poured and must stay in its original containers (Technical Violation given). All resident rooms were observed to be clean and have the proper linens. Toxins were observed to be stored inaccessible to residents. There was an appropriate supply of cleaning products, linens, hygiene products and paper products available for residents. Bathrooms were equipped with necessary grab bars, and non-slip floors/mats were present. Hot water temperature for one sink measured at 122 degrees and second sink at 109 degrees. LPA recommended admin to lower water heater to make sure both sinks are found to be within Title 22 regulations of 105 to 120 degrees Fahrenheit. Facility's fire extinguisher was observed to be charged and last inspected on 09/2024( Fire Dept to come do inspection). Smoke and carbon monoxide detectors were tested and operational. Disaster drill last conducted on 07/05/2025. Continued to 809C..... continued from 809.... LPA conducted a file review check for 3 out of 3 residents. All documentation found to be in file. LPA discussed with admin having Admission Agreements under new license name. LPA conducted staff resident review for 2 staff. Staff currently in training and had conversation with admin to keep all certifications in file once completed. CPR for one staff expired as of 1/25 and another staff not able to provide certification ( Technical Violation, given). CPR Certificates for all staff to be sent by 12/09/2025* Proof of fire extinguisher has been inspected for 2025* No deficiencies cited during todays visit. Exit interview conducted. Copy of report discussed and provided to Licensee. .
