What the inspector wrote

Licensing Program Analyst (LPA) Contreras arrived unannounced to conduct a pre-licensing inspection and was greeted by applicant/ current administrator(admin) Anirudh Wadhwa. Facility is an Adult Residential Facility for the elderly in process for a Change of Ownership. Facility currently has 21 residents in care. LPA and admin toured the indoor and outdoor portions of the facility. Facility was found to be clean and at a comfortable temperature. All required postings found to be visible. Activities calendar visible. All resident bedrooms were found to be in clean and good repair. All had required chair, chest drawer and beds were made with appropriate linens. One resident bedroom lacked a window screen. Restrooms had grab bars and bath mats. Sun room is currently under construction for resident activities. LPA advised admin to have an outdoor area available for residents. Admin cleared a second outdoor area to be free from hazards for residents during inspection visit. LPA observed there to not be emergency food and emergency water supply. Admin ordered emergency food supply during inspection and brought water supply to meet the 72 hour regulation. LPA observed there to be perishables to meet Title 22 Regulations. There were canned goods and non-perishable foods in pantry. Refrigerator and freezer were functioning and operational. LPA observed foods in freezer to not be labeled or have expiration dates. LPA had conversation with admin that all food in freezer and refrigerator must be labeled with expiration dated noted. LPA observed there to be plates and utensils including additional kitchenware. All cleaning products, sharp items, other toxins and chemicals are locked and inaccessible for residents. There are an ample amount of extra linens, towels, hygiene products. LPA observed fire extinguisher charged and last inspected on 9/25/2025. Fire alarm and carbon monoxide alarms are inspected through Fire Marshall with a last inspection conducted on 9/4/2025. Signal system on all doors leading out of facility were on, tested and found operational. First aid kit available. Continued onto 809C.... continued from 809.... Water was measured in faucets used by residents and was not found to be within regulation between 105 & 120 degrees F. Admin lowered water heater, LPA conducted a second water measurement which was found to be in regulation in all 3 bathrooms accessible to residents. LPA discussed with admin keeping a temperature log check to ensure water is not over 120 degrees in all faucets. LPA conducted a medication spot check and observed that all medication is centrally stored and locked in newly constructed medication room. Centrally Stored Medication Log (CSML) is up to date. LPA conducted a file review for residents. All documentation including Medical Assessment found to be up to date and in file. LPA conducted a staff file review. All documentation found to be up to date. CPR and training up to date. Administrator Certificate: Anirudh Wadhwa #7016455740 Expires:7/24/2026 COMP III has already been discussed with applicant/current administrator in previous visit conducted on 2/05/2026. Items Addressed and Discussed during Inspection: - Expiration dates must be kept on all food items - One window screen missing in resident room ( Will send picture of new window screen) - Water temperature must be regularly checked to be at 105-120 degrees F (Log will be made for temperature checks) - Outdoor area must be available for residents (Outdoor cleared during visit) - Emergency food and water needs to be in facility (Ordered during visit w/ proof of receipt) Pre-Licensing completed. Facility is ready to be Licensed. LPA will submit Pre-Licensing Application Report to the Application Unit Analyst in Sacramento. Application Unit Analyst will notify Applicants of Status. Report given and read with applicant. No deficiencies given during today's visit.
