What the inspector wrote

Continue from LIC9099... The payee services have voided their original check issued for a full month of board and care fees, and re-issued a new check for R1's nine day stay prior to been relocated to a different facility. The letter was requesting if the facility could issue a refund of $1420.07 and made payable to R1 with a PO Box mailing address located in Ukiah. According to Licensee, they were not aware where R1 was residing until they received a call from R1's case worker and letter from R1's representative of their payee services inquiring for the refund, and providing an address to mail the refund, so the facility on 10/22/25 mailed R1 the requested refund check #9412 in the amount of $1420.07 to the mailing address provided with a memo stating refund and issued to R1. Although it is unclear if Licensee had knowledge or not of R1's relocation address, LPA was provided with copies of pertinent documents that indicates that the facility have mailed the refund to R1 two days prior to LPA's visit. A finding that the complaint allegation of facility has not provided due refund as required is unsubstantiated meaning that although the allegation may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is UNSUBSTANTIATED.
