What the inspector wrote

Licensing Program Analyst (LPA) Alviso arrived unannounced to conduct a Required -1 Year visit, at approximately 10:20am on 8/5/25, and met with Administrator Otilia Molina Vazquez. Administrator Otilia works for licensees other home, Wild Rose Care Home. Licensee Mary Garcia, and facility's Administrator Erika Molina arrived to meet with the LPA. Facility has an approved dementia plan of operation. There is an approved hospice waiver for three (3) residents. Facility has a required emergency disaster plan and a required infection control plan. Fire clearance is approved for six (6) non-ambulatory, which includes one(1) bedridden. Facility has required smoke alarms, ten (10), which are also carbon monoxide detectors. Fire extinguishers, three (3) were serviced and tagged as required. Emergency disaster drills are being conducted as required, per file review. LPA reviewed six (6) resident files. All files were complete. LPA reviewed four (4) staff files. All staff have criminal record clearance. All staff have required first aid certification and CPR certification. All staff have required annual training. LPA toured the facility with Administrator Otilia. Hot water was measured at 116. degrees Fahrenheit. All exits were free and clear of obstruction. Food supply was observed to be sufficient. All medications were locked and inaccessible to residents in care. All cleaners/disinfectants were locked and inaccessible to residents in care. Continued on LIC809C.. Facility had sufficient linens, furnishings, paper products, and personal protective equipment (PPE). Facility had sufficient lighting in all rooms of the facility for resident use. Shower bathroom had grab bars and non-slip/non-slip textured flooring in the shower. All bathrooms had grab bars for residents use. Facility was clean and orderly during the inspection LPA is requesting the following documents be updated and submitted by 9/5/25: LIC308 - Designation of Administrator Responsibility LIC500 - Personnel Report LIC610-Emergency Disaster Plan (9 pages)- if any updates submit a copy Copy of Current Liability Insurance Infection Control Plan- if any updates submit a copy Copy of current Administrator Certificate There were no deficiencies cited during today's inspection. Exit interview was conducted with Administrator Erika Molina.
