What the inspector wrote

Licensing Program Analyst (LPA) Christi Coppo arrived unannounced to conduct a required Annual inspection and was greeted by Administrator Raymond Dichoso. Administrator certificate #7005427740 expires 8/31/26. Facility currently has five [5] residents four [4] of which are on hospice. At approximately 9:30am LPA toured the building and grounds. The facility was found to be clean and at a comfortable temperature. LPA observed at least a 2 day supply of perishable and 7 day supply of non-perishable food. Food was found to be stored in a safe manner with open items covered. Kitchen cabinet containing cleaning supplies was locked. Kitchen drawer with sharp knives locked. All bedrooms were equipped with lighting, night stand, and chest of drawers. All bedrooms were clean and in good repair. Extra hygiene products and linens were available. Resident bathroom had required bath mat and grab bar. Water temperature in sink accessible to residents in care measured at 112.6 degrees F in room #1/2, 112.9 degrees F in the back bathroom, and 113.5 in the kitchen which are all within the allowable range of 105 to 120 degrees F. Ramp on west side of house out of room #5 is in disrepair. The wood planks at the bottom third portion of ramp can still bear weight, but they bow and crack when LPA and Admin step on them. Wood planks have many cracks and the ground underneath can be seen through the cracked wood. The planks have worn and cracked off in many areas. The very top of the ramp was observed to be stable and not in disrepair. Fire extinguishers were last inspected 4/24/25. Smoke/Carbon Monoxide detectors located throughout the Continued on 809C... Continued from 809... facility were tested and operational, fire door closed properly once alarm activated. Facility’s last quarterly disaster drill was conducted 1/3/26. Facility has a backup generator for use during a power outage. At approximately 10:30am LPA conducted review of five [5] staff records. Last year, LPA and Admin discussed training and training materials. LPA observed Admin to have followed through with all discussion points from last year in entirety. Admin chose an approved vendor. All training completed, all required subject matters covered; training certificates present for all staff. All staff have current FirstAid/CPR. All required documentation present. At approximately 11:00am LPA conducted a review of five [5] resident records. All required documentation present. At approximately 12:30pm LPA and Admin conducted a spot check of medication and medication records. Medication is centrally stored in a locked cabinet. LPA and Admin discussed all current physician's orders for medications should be listed on the Centrally Stored Medication Log (CSML). All medications should be listed on the CSML, including PRNs. LPA and Admin discussed hospice comfort medications and hospice medication orders. All medications on current doctor's orders must be present in the facility at time of resident's move in. LPA and Admin discussed ensuring all medications are present at time of move in, if a resident's family member can provide the medications at move in, that is allowed. But if they have not provided them by time of move in, Admin will purchase the medications to ensure that residents have all medications present that are listed on their current doctor's orders. Updated copies of the following documents were requested for facility file and are to be submitted to CCL within 30 days of this visit: LIC500- Personnel Report Liability Insurance No deficiencies cited.
