Licensing Program Analyst (LPA) Erik Gonzalez Campos arrived unannounced on 3/3/2022 to conduct a case management inspection. This inspection is a follow up to an SOC341 received by community care licensing on 2/8/2022. LPA met with Morgan Holein, executive director and Katelyn Ledesma, Health and Wellness Director. LPA reviewed resident file and assisted living resident and services agreement. LPA and health and wellness director had a conversation about who will be the administrator for the facility. Mariele Soriano is still the administrator but will be replaced by Morgan Holein. LPA requested the following documents during the visit. LIC 200 LIC 308 LIC 500 LIC 501 or Administrator’s Resume Administrator Certificate Minutes of meeting appointing new administrator Exit interview conducted with administrator and a copy of this report emailed to the facility.
