What the inspector wrote

Licensing Program Analyst (LPA) Erik Gonzalez Campos arrived unannounced on 12/28/2022 to conduct a case management inspection. LPA met with administrator, Lydia Gravelyn. Case management is regarding a self reported SOC341 received by Community Care Licensing on 12/20/2022. Facility has conducted an internal investigation and reported incident to the Santa Rosa Police Department. LPA will follow up by requesting police report. No deficiencies cited during today's inspection. Exit interview conducted with administrator and a copy of this report emailed to the facility.
