Case management visit made out to this facility on 05/26/2023 by Licensing Program Analyst (LPA) Charlie Yang who was met by the facility designated Administrator, Steven Paul Hardin, who was briefly interviewed. Current census was 12 residents. The purpose of this visit was to deliver the forms and documents associated to the Informal Conference that was held back on 05/23/2023. The forms and documents were reviewed with the facility designated Administrator at this time. No deficiencies were observed or cited during today's case management visit. Exit Interview
