What the inspector wrote

On 08/04/2025, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to conduct a case management visit. LPA met with Facility Designated Administrator (FDA), Kashmindar Kaur and explained the purpose of the visit. The purpose of this visit was to follow up on an incident reported by the facility on 07/30/2025. Current census was 32. On 07/30/2025, Resident 1 (R1) informed a facility staff member that they had given their food stamp card to Staff Member 1 (S1) to help purchase groceries for S1's family. R1 reported that S1 used the card for a total of approximately $700.Additionally, R1 stated that they provided S1 with their credit card number after S1 requested to place a DoorDash order. S1 claimed the card was used only once; however, a review of R1’s bank statements revealed eight separate DoorDash transactions between June 14, 2025, and July 7, 2025. When R1 confronted S1 about the charges, S1 said they would help investigate the matter but failed to follow through. During this visit, LPA Pascua interviewed staff and obtained documentation relating to this incident. Based on interviews, and observations made during this visit there are no deficiencies observed during this visit. The department will come at a later time if further follow up is needed. An exit interview was conducted and a copy of this report was provided to the facility at the end of this visit.
