What the inspector wrote

On 02/27/2026, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct an annual visit. LPA Pascua met with Facility Designated Administrator (FDA), Cynthia McDaniel and explained the purpose of the visit. This facility is licensed to serve 40 residents who are 60 and older. 29 of those residents maybe non-ambulatory. The physical plant also has 4 separate buildings under the license. In addition, the facility has a hospice waiver for 2. Current census was 21. A brief interview with FDA McDaniel was conducted. LPA Pascua reviewed 5 staff files and 5 resident files. All files were complete and up to date. The facility designated administrator (FDA), Cynthia McDaniel has an active administrator certificate #7005085740 and expires on 06/18/2026. A tour of the facility was conducted. This facility has 4 separate buildings. The main building houses the facility kitchen and dining area. LPA Pascua identified kitchen supplies, and perishable and non-perishable food supplies. There is also a basement in this main building that houses additional non-perishable food supplies, laundry area, and linens. Building #2 houses 3 resident suites and an additional storage unit with several cleaning supplies and refrigerators. Building #3 houses 4 resident bedrooms, a bathroom, and office area. Building #4 was toured with a kitchenette and washer and dryer for resident use. Resident bedrooms were reviewed and contained sufficient components to meet resident needs. Resident bathrooms were observed, hot water was measured to ensure within Title 22 guidelines. All other areas intended for resident use were observed and in good repair. Fire extinguishers throughout the facility were identified and serviced by the Local Fire Extinguisher company 08/12/2025 and expires on 08/12/2025. This facility holds a locked medication cart. Along with the staff responsible in handling medication, LPA Pascua reviewed and compared medication with medication dispensing logs. It was learned that there are 2 residents who use injection insulin, however are able to conduct this themselves. In addition, LPA Pascua provided new medication guidance regarding pre-pouring medication. LPA Pascua observed two containers in which were identified by staff as containers used to place medication in prior to administration. LPA Pascua informed staff that this was no longer allowed and provided best practices. LPA Pascua has printed out the updated RCFE Medication Guide for this facility during this visit. Smoke detectors and carbon monoxide were observed and serviced by Jorgenson Company on 02/04/2026. A tour of the outside grounds was conducted. It was observed that there were no hazards present. Gates and perimeter fence were in good repair. The following forms were requested to be submitted and reviewed by this LPA: -LIC 308 -LIC 400 -LIC 500 -LIC 610E Technical Assistance was provided today for Section 87465(h)(5). Based on the observations made during today's visit, there are no deficiencies were observed or cited during this annual visit. An exit interview was conducted and a copy of this report was given to Facility Designated Administrator.
