Unannounced case management visit made out to this facility on 02/09/2023 by Licensing Program Analysts (LPAs) Charlie Yang and Kimberly Viarella who were met by the facility designated Administrator Cecilia Candido. Brief interview was conducted with the facility designated Administrator at this time. Current census was 4 residents. Brief tour of this facility was conducted. Garage area was toured. Side yard and backyard area was toured alongside the facility designated Administrator Cecilia Candido. Brief discussion was held with the facility designated Administrator in regards to the overall facility and the requirements for the facility Licensee/Administrator to maintain compliance at all times.
