What the inspector wrote

Licensing Program Analyst (LPA) Renee Campbell and arrived at the facility to conduct an unannounced annual inspection on 06/04/2025.  LPA Campbell met with Jason Trinidad, General Manager and explained the purpose of the visit. Upon entry, LPA Campbell observed one resident in the living room watching TV and another resident in the dining room finishing breakfast. LPA Campbell inspected the physical plant including but not limited to the common area, kitchen, dining area, client bedrooms, client bathrooms, laundry room and outside courtyards of the facility to ensure compliance with Title 22 regulations. The community consists of a two story building licensed to service 15 ambulatory residents, of which there may be 6 non-ambulatory residents. There is an approved hospice waiver for 5 hospice residents. The second story of the building is reserved for offices , additional food pantry and miscellaneous storage space. Residents live on the first floor of the building where there is a living room and dining room. There are 3 bathrooms and 10 bedrooms. LPA Campbell observed the facility to be free of odor, clean and in good repair. LPA Campbell observed bedrooms to be properly furnished with appropriate bedding and lighting. There are no bodies of water present. LPA Campbell observed sufficient seven-day non-perishable and two-day perishable food supplies. Fire extinguishers, smoke and carbon monoxide detectors are in good repair. The facility thermostat was observed at 75 degrees Fahrenheit. LPA Campbell checked medication storage and found medication to be locked away and inaccessible to clients. The First aid kit was complete as well. LPA requested client and staff files for review. LPA reviewed 4 of 8 resident files and 5 of 9 staff files. Of the 4 resident files reviewed, all files were complete. Of the 5 staff files reviewed, all files were found to be complete. Toxins were made inaccessible to clients in care. Toxins are stored and locked in the laundry room. The fire extinguisher was fully charged and was last inspected on 09/26/2025. Weekly menus were observed in the kitchen along with a shower and laundry schedule for residents. Staff provided documentation of smoke alarm testing completed by Siemens Industry Inspection Services. The smoke alarm systems passed inspection. Staff tested the carbon monoxide alarm and it was found to be functioning as well. Per California Code of Regulations, Title 22, no deficiencies were observed during today’s visit.
