On 10/21/2024, Licensing Program Analyst (LPA) Arielle Pascua arrived unannounced to this facility to conduct a case management visit. LPA met with Licensee, Marinela Placintar and explained the purpose of the visit. Current census was 5. A brief of interview with Licensee Placintar was conducted. The purpose of the visit was to follow up on an incident that was reported by the Licensee to the department on 10/21/2024. During the course of this visit, LPA obtained the following information for R1: -ID/Emergency Contact Sheet -Pre-Appraisal -Needs and Services Plan -Hospice Care Plan -Any and All incident reports pertaining to R1 As a result of this visit, no deficiencies were observed or cited during this annual visit. An exit interview was conducted and copy of the 809 and 809-C was provided to Licensee, Marinela Placintar.
