What the inspector wrote

On 8/24/22/22, Licensing Program Analyst (LPA), M. Yang arrived unannounced to conduct a Case Management - Deficiencies inspection. LPA introduced self, stated the purpose of the visit and met with the Administrator Crystal Gonzales The purpose of the visit is to address incident that occurred on 06/01/22 where Staff 1 (S1) removed resident 1 (R1) lotions and after shave without resident’s permission. A deficiency is being cited on the attached 809D in accordance to California Code of Regulations, Title 22, Division 6. An exit interview was conducted, and a Plan of Correction was reviewed and developed with administrator. A copy of this report and appeal rights was provided to Administrator.

At 12:10 PM, LPA and Administrator observed five cleaning chemicals stored under sink in the activity room unlock accessible to residents in care this poses an immediate health, safety or personal rights risk to persons in care.
