Licensing Program Analyst (LPA) K. Kaur arrived at the facility unannounced for initial 10-day complaint inspection. LPA discussed the purpose of the visit and the elements of the allegations with administrator. LPA delivered the following findings. Based on records reviewed and interviews conducted Staff 1 was hired and worked at the facility until it was discovered staff’s criminal background check was not cleared and staff was terminated. The preponderance of evidence standard has been met, therefore the above allegations are found to be SUBSTANTIATED. See citations on the attached LIC9099D and assessed civil penalty. Exit interview was conducted and appeal rights were provided.
