What the inspector wrote

Licensing Program Analyst (LPA) Shawna Doucette arrived at the facility unannounced to conduct a case management for an incident that occurred on 5/15/25. LPA met with Administrator Mandy Rancour. LPA reviewed R1's LIC 602. R1 is unable to manage her own medications. LPA interviewed Administrator. R1's 8 AM pills were administered however facility staff did not ensure R1 swallowed her pills. R1's pills were found by staff in the afternoon. R1 has a history of not swallowing administered pills. An exit interview was conducted with the Administrator and a copy of this report was provided with appeal rights and plan of correction.

The plan shall encourage routine medical and dental care and provide for assistance in obtaining such care, by compliance with the following: (4) The licensee shall assist residents with self-administered medications as needed. This requirement was not met as evidenced by: Licensee did not ensure R1 swallowed administered AM medication which was later found in the afternoon which poses an immediate health safety and or personal rights risk.

    87211 Reporting Requirements (a) Each licensee shall furnish to the licensing agency such reports as the Department may require, including, but not limited to, the following:2) Occurrences, such as epidemic outbreaks, poisonings, catastrophes or major accidents which threaten the welfare, safety or health of residents, personnel or visitors, shall be reported within 24 hours either by telephone or facsimile to the licensing agency and to the local health officer when appropriate. This requirement was not met by: Licensee did not report R1's contagious prohibited condition, which poses a potential health, safety and or personal rights risk to residents in care.
