On 10/13/2025, Licensing Program Analyst (LPA) M Vega conducted an unannounced inspection at the facility and met with Administrator - Mandy Rancour. The purpose of the visit was to close a complaint investigation and deliver findings regarding the above allegation. It was alleged that the facility Staff speaks inappropriately to resident. (R1-R5 see attached confidential names list). Based on interviews and record review it has been determined that the facility does speak to residents in a respectful and appropriate manner and determined the allegation is unfounded. This agency has investigated the complaint alleging “Staff speaks inappropriately to resident .” We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened or is without a reasonable basis. We have found that the complaint was unfounded, therefore we have dismissed the complaint.
