What the inspector wrote

This an amended report. On 3/11/22, Licensing Program Analyst (LPA) M. Yang arrived unannounced to conduct complaint investigation visit and deliver complaint finding on the above allegation. LPA introduced self, stated the purpose of the visit, and met with Administrator Martin Vale. During the course of the investigation, LPA conducted interviews with staffs and residents. Based on interviews conducted, it could not be proven or disproven that S1 and S2 spoke disrespectfully to R1. The preponderance of evidence standard has not been met, therefore the above allegation is found to be UNSUBSTANTIATED. Exit interview conducted. As a COVID-19 precautionary measure, a copy of this report will be provided via email. Report signed on-site.
