What the inspector wrote

On 9/03/2025, Licensing Program Analyst (LPA) M. Medina conducted an unannounced Annual Inspection. PA introduced self, stated the purpose of the visit, and was granted entry to the facility by direct care staff. Administrator Jacklyn Becerra contacted by telephone and arrived a short time later to conduct facility inspection with LPA. LPA conducted facility tour with Administrator. Facility observed to be clean, odor free, and a comfortable temperature. Facility tour began in resident bedrooms. All bedrooms observed to have all required furnishings. All common areas of facilities observed to have adequate seating available for residents in care. Kitchen toured, LPA observed a 7-day supply of non-perishable foods and a 2-day supply of perishable foods. All knives observed to locked and secured. Facility laundry room is located within office area, laundry room was observed to be locked and secured. Facility also has chemicals stored in laundry room. Bathrooms observed to have shower chairs, skid resistant mats and grab bars available. Medications are locked and secured in medication cart. Fire extinguishers present in hallway near bedrooms, in kitchen, and near back exit door near office, all fire extinguishers have a service date of 6/06/2025. Facility is equipped with sprinkler system. LPA observed adequate supply of linens available in hallway cabinet. LPA observed sufficient seating under covered patio area in the back of the facility. Side gate was self-closing and self-latching. LPA requested the following documents to be submitted to Fresno Regional Office no later than 9/12/2025: Copy of Liability Insurance, Personnel Roster (LIC 500), Register of Facility Residents (LIC 9020), Designation of Administrative Responsibility (LIC 308). No deficiency observed. Exit interview conducted with Administrator. A copy of report provided for facility records.
