What the inspector wrote

On 05/29/25, Licensing Program Analysts (LPAs) L. Salazar and M. Vega arrived at the facility unannounced to conduct a case management visit based on incidents reported. LPAs were greeted by Administrator, stated the purpose of the visit and were allowed entry into the facility. LPAs met with Administrator, David Shellhamer. LPA requested resident records and obtained copies of information for Resident R1, Resident R2, Resident R2 and Resident R4. Due to time constraints, LPA will review records and return to the facility at a later date. No Deficiencies cited on today's visit.
