Facility-requested visit·April 22, 2026

On 04/22/26, Licensing Program Analyst (LPA) M. Yang arrived at the facility unannounced to conduct the inspection for finalizing building operation on building B. LPA met Administrator (A1) Gustovo “Gus” Chavez. LPA conduct tour of facility building B with A1. Building B was observed to be at a comfortable temperature, clean, and no passageway obstructions or fire hazards. Building B will consist of mild to severe memory loss to memory care unit. Exits in Building B were observed to have a 30-second delay egress. Fire sprinklers observed installed throughout building. Fire extinguishers observed throughout building with service date 11/14/25. All bedrooms observed with no obstruction. All bathrooms observed with nonskid mats and grabbed bars. At this time, Building B is currently ready for placement of residents. Exit interview was conducted. A copy of this report was provided to Administrator.
