87555(b)(27) General Food Service Requirements. All kitchen areas shall be kept clean and free of litter, rodents, vermin and insects.This requirement is not met as evidenced by: Based on interviews and observations, the licensee did not comply with the section cited above, as there were rodent droppings observed in the kitchen and staff admitted that the facility had an issue with rodents in the kitchen, which poses and immediate health and safety risk to persons in care.

All window screens shall be clean and maintained in good repair.This requirement is not met as evidenced by: Based on observation, the licensee did not comply with the section cited above, as the window screen in the theater room was missing, which poses an immediate health and safety risk to residents in care.
