What the inspector wrote

Licensing Program Analyst (LPA) Teresa Camara conducted a case management - deficiencies visit due to a deficiency observed during the course of a complaint investigation of complaint control number 29-AS-20230418134450. LPA conducted interviews regarding the above noted complaint with resident 1 (R1) at 1:15 p.m., staff 1 (S1) at 1:50 p.m. and staff 2 (S2) at 1:55 p.m. During the interviews it was found that an incident took place on or about 3/31/2023 in which cash and a debit card were stolen from R1's purse which was kept inside R1's closet. Unauthorized transactions were discovered to have been made with the stolen debit card. S2 stated it was their understanding R1's family reported this to the facility's community relations director and/or the facility's Resident Care Director, both of whom are working on conducting further investigation into the allegation. The report from the family to facility staff was received on or around the first week of April 2023. LPA reviewed incident reports received by the Department from the facility, however no incident report was received regarding this incident. An SOC341 was received by the Department, however that was not filed by the facility. Pursuant to Title 22 Division 6 Chapter 8 of the CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D). Exit interview conducted. Appeal Rights Discussed. A copy of the report was provided. Civil penalty assessed.
