What the inspector wrote

Licensing Program Analyst (LPA) KaSandra Lopez conducted an unannounced Case Management - Deficiencies inspection at the facility today due to a deficiency observed during a complaint investigation. Record review and interview with Business Office Manager Jennifer Miller, revealed Staff #1's (S1) criminal record clearance was not transferred and associated to the facility. File review and an interview with S1 revealed S1 has worked at the facility since August 2022. During the inspection, Ms. Miller associated S1 to the facility. This deficiency warrants an immediate civil penalty. This is also a repeat violation of the same citation that was issued on 03/22/2022 which is within a twelve (12) month period. Therefore, civil penalties are being assessed in the amount of one hundred ($100) per day for the maximum amount of thirty (30) days. The following deficiencies were observed (See LIC 809-D.) and cited from the California Code of Regulations, Title 22 and California Health and Safety Code. Exit interview and report reviewed with Jennifer Miller, Business Office Manager. A copy of the report was provided.

This requirement is not met as evidence by: Based on interview and record review the licensee failed to comply with the section cited above as Staff #1 (S1) has worked at the facility since August 2022 and their criminal record clearance has not been transferred to the facility which poses an immediate health and safety risk to residents.
