What the inspector wrote

Administrator stated that the van has not been working since August 2020. A review of R1's records starting at 11:47 am revealed that they will plan, arrange and/or provide transportation to medical and dental appointments as follows: we will make the appt, we will take resident. Based on the information obtained during the course of the investigation this allegation is deemed substantiated at this time. Exit interview conducted. Today's reports and appeal rights were reviewed and emailed to Administrator.
