What the inspector wrote

Licensing Program Analysts (LPAs) Teresa Camara and Kelly Dulek conducted a Case Management-Deficiencies visit to issue citations for deficiencies observed during the investigation of complaint control #29-NP-20210826165010, unrelated to the complaint allegations. During the facility tour the LPAs conducted along with Assistant Administrator Jennifer Sharma at 10:28AM, LPAs observed an unlocked closet labeled "Telephone & Cable" at 10:32AM. Additionally, at 11:05AM, a resident restroom/toilet was observed to be dirty. Pursuant to Title 22, CA Code of Regulations, the following deficiencies were cited (refer to LIC 809-D). Exit interview conducted, report issued, and appeal rights provided via email.
