What the inspector wrote

Regarding the allegation: Staff did not ensure facility was free from pests. On 11/04/2025, the Department received a complaint alleging that two (2) rooms had bed bugs. Interviews conducted with the ED and staff revealed bedbugs were observed in Rooms #145 and #146. Interviews with the ED and staff reported inconsistent dates of when the bed bugs were first observed. The ED explained that he was made aware of the bed bugs on 11/07/2025 and reached out to a pest control company the same day. Per record review and interviews with staff and the ED, Western Exterminator Company began “Bed Bug Heat Treatment” on 11/10/2025. The ED explained that the residents affected received new mattresses and cleaned bed sheets. The ED explained that the surrounding rooms were also inspected and sprayed to ensure that the bed bugs did not spread any further. Interviews with staff reported that they do regular room checks and if any bed bug activity is noticed they report it to management. The ED provided the LPA a copy of the invoice/ receipt of Western Exterminator Company dated 11/10/2025. Based on the information provided by interviews and record review, the preponderance of evidence standard has been met, therefore the above allegation is deemed Substantiated at this time. Per the California Code of Regulations, Title 22, Division 6, Chapter 8, the following deficiency was observed and cited during the visit (See 9099-D). Exit interview conducted. A copy of the report and appeal rights were provided.
