Licensing Program Analyst (LPA) Angela Barutyan conducted an unannounced case management - incident visit at 12:07PM. Upon arrival, LPA met with staff and Executive Director (ED) Zachary “Zak” Howell and reason for the visit was explained. Entrance interview conducted. On 06/26/2025, the Department received an SOC 341 stating that on 06/25/2025, a family member reported to the facility that Staff #1 (S1) verbally abused Resident #1 (R1) and other residents on 06/08/2025 and various unknown days. Another staff member, Staff #2 (S2), supported the allegation. The facility cross-reported to the Ventura County Sheriff and the Long-Term Care Ombudsman. During today’s visit, LPA conducted a brief physical plant tour, interviewed ED, attempted interviews with one (1) resident, one (1) staff, and one (1) family member, and reviewed and obtained copies of pertinent documents relevant to the investigation. Prior to issuing final licensing report, it has been determined that further investigation is needed at this time. Exit interview conducted. Copy of the report was provided.
