What the inspector wrote

Licensing Program Analyst (LPA) Esther Cortez arrived to the facility to conduct an unannounced Case Management – Deficiencies visit at the facility today. Upon arrival, LPA met with staff and explained the reason for the visit. Administrator Roberto Dela Vega Ramirez arrived at approximately 11:30 a.m. and the reason for the visit was explained. Entrance interview conducted. During today’s visit, the LPA conducted a file review of Resident’s 1 (R1) records. R1’s file review revealed that R1’s Admission Agreement noted that if the resident is admitted any day after the first of the month, payment will be based prorated based on the remaining days of the month. However, R1 moved into the facility on 10/16/25, and was overcharged and paid the total monthly cost of Basic services of $6500. Interview with the Administrator revealed that the Resident’s monthly fee would be due every 16 th of every month since the amount was not prorated. However, there is no documentation to confirm their statement. Pursuant to Title 22, California Code of Regulations, the following deficiencies were cited (refer to LIC809-D). Exit interview conducted and copy of the report and appeal rights were issued .
