What the inspector wrote

Licensing Program Analyst (LPA) Martha Arroyo conducted a Case Management - Deficiencies visit in conjunction with a complaint visit (Complaint Control # 29-AS-20230801143754). The purpose of the visit is to issue a citation for a deficiency observed during the complaint investigation. During the complaint investigation of complaint # 29-AS-20230801143754, the following deficiency was observed: On 08/04/2023, records review revealed that Resident #1’s (R1’s) centrally stored medication and destruction record (CSMDR) is not being updated with medication expiration and start dates. Interview conducted with staff revealed the CSMDR’s are not being completely filled out for all residents’ centrally stored medication. Pursuant to CCR, Title 22, Division 6, Chapter 8, the following deficiencies are cited (Refer to LIC LIC9099-D). Failure to correct citations can result in civil penalties. Exit interview conducted. Citation issued. A copy of the Appeal Rights and Report was issued.

  • Keep bath, laundry and kitchen floors clean

    Maintenance shall include provision of maintenance services and procedures for the safety and well-being of residents, employees and visitors.Floor surfaces… shall be maintained in a clean and odorless condition. This requirement is not met as evidenced by: Based on LPA observation during the facility walkthrough, the licensee did not comply with the section cited above, as R1’s bedroom has a lingering smell of pet urine, which poses a potential health and safety risk to residents in care.
