What the inspector wrote

Licensing Program Analyst (LPA) Jill Nakagawa conducted an unannounced 1-Year Required inspection at Six Roses Senior Care, on May 6, 2026 at 2:00 PM. LPA met with Administrator/Licensee Salvador Ivan Hernandez. The facility currently has 4 residents in care. All 4 residents were happy to be there and appeared clean and appropriately dressed. There was one staff and the Administrator on site. Administrator conducted a walk through of facility and LPA observed client rooms were furnished per regulations. LPA observed all walkways and exits to be unobstructed. Facility has at least two days of perishable and one week of nonperishable foods. Facility has space indoors and outdoors for client activities. Administrator has a lively activities program, including outings and celebrations. Disinfectants/toxins are kept locked in a closet and inaccessible to clients in care. Extra linens and paper supplies are available for clients. Medications are kept locked and documented on a Centrally Stored Medication Log. LPA observed fire extinguisher in kitchen which was charged and purchased on 03/2026. The facility has 1 operational carbon monoxide detector and 7 interactive smoke detectors which were tested and operational. All exterior doors had operational auditory alarms. House temperature was 73 degrees F and water temperature was 117.6 degrees F, which is within regulation. Facility has 30 days supply of PPE and residents have a 30 days supply of medication and incontinence supplies. LPA inspected 4 resident and 3 staff files and found them to be complete. Administrator has arranged for an in-person First Aid/CPR course for staff. LPA requested an updated LIC500 and proof of Liability Insurance. LPA and Administrator/Licensee discussed in-person first aid/cpr training and updating 602's and reviewed Needs and Services Plans, Emergency Disaster Plan and Infection Control Plan. No deficiencies cited at today's inspection.
