Inspector’s narrative

What the inspector wrote

Based on CCLD records and LPA Cheng records, LPA Cheng determined that the above allegation did occur. Facility provided an incident report dated 2/25/2021 and informed that they had faxed it to CCLD the same day. CCLD Chico Regional Office has no record of receiving the fax. CCLD uses an electronic system for incoming faxes. LPA Cheng was also the duty officer on 2/25/2021 and has no e-mail records of any incident reports from the facility being forwarded to the assigned LPA Mai Thao. LPA Cheng spoke to Mai Thao and was informed that the facility file in the CCLD Chico Regional Office shows no record of having the incident report dated 2/25/2021. Based on LPA information obtained during investigation, the preponderance of evidence standard has been met; therefore, the above allegation is found to be SUBSTANTIATED . California Code of Regulations (Title 22, Division 6, Chapter 8), are being cited on the attached LIC 9099D. Exit interview conducted and a copy of the report along with appeal rights were given.
