Inspector’s narrative

What the inspector wrote

Licensing Program Analyst (LPA) Hiratsuka conducted this unannounced prelicensing visit. LPA toured with Isabel Saunders, Resident Care Director. This facility is undergoing a change in ownership. There are 50 residents in care. This facility has a fire clearance for 72 non-ambulatory with six whom may be bedridden. The main entrance opens to a small foyer that leads to the main front area of the facility. There are administration offices to the left of the main entrance. There is a memory care unit with delayed egress and its own courtyard. This is a two story building. There is a laundry room residents may use on the second floor. Main laundry room is on the first floor. There is a salon that is locked when not in use. There is one elevator and several sets of stairs. The kitchen was inspected. LPA learned during this visit there was a medication error that started March 2026 and was not discovered until today. Because of the error the facility does not pass the prelicesing inspection. LPA spoke to Administrator April Kale and stated Licensee is to submit a written plan of correction and proof of training. When that has occurred LPA is going to clear the building to be licensed. Component III orientation was waived because the licensee owns several other facilities and the administrator works at another building.
