What Happened

Case management investigation of self-reported incident from 7/10/2024. Child was able to exit classroom by reaching over short gate and opening door, then able to reach exit door to sidewalk. Staff member entering facility prevented actual exit to sidewalk. Type A zero-tolerance violation cited with immediate $500 civil penalty.

Observations

  • Child opened classroom gate by reaching over (gate too short)
  • Child opened door to off-limit area
  • Child reached exit door to sidewalk and opened it
  • Staff member entering facility prevented child from exiting to sidewalk
  • No injury occurred
  • Type A deficiency - zero tolerance violation
  • Violation of CCR Title 22 101229(a)(1)
  • Immediate $500 civil penalty assessed
