What Happened
Case Management incident investigation. Preschooler left unattended in bathroom for approximately 5 minutes during classroom-to-playground transition. Staff failed to complete name-to-face roll call or classroom/bathroom sweep. Type A violation issued. Teacher terminated effective 04/19/2024. Civil penalty of $500 assessed.
