What the inspector wrote

On February 18, 2026, Licensing Program Analysts (LPA’s) Jyoti Saini and Kassandra Medrano met with Director Rina Jarin for an unannounced case management inspection in response to an unusual incident that the facility self reported to the regional office (RO) on 01/18/2026. Present in the facility are 28 children, and 6 staff including the director. On January 18, 2026, the RO received information that a child’s services were terminated by the facility. The child’s family was notified in writing that services were discontinued due to health and safety concerns. The written communication indicated that specific details regarding the concerns could not be discussed. During the review, the LPAs examined documentation related to the termination. The facility did not provide documentation demonstrating that the parent was informed of the specific unusual incident or event that allegedly posed a threat to health and safety. Based on record review, the Licensee did not ensure that the child’s authorized representative was informed of an unusual incident or event that threatened the health or safety of the child. This is a violation of California Code of Regulations, Title 22, Section 101212(f) is cited on the attached LIC 9099D. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted, report and appeal rights were reviewed with the Facility representative, Rina .
