What Happened

Follow-up case management investigation of self-reported incident from 10/1/2024. Sufficient evidence found that staff member (S1) handled children roughly, including pulling child's hair. Staff member no longer employed at facility. Type A violation cited requiring licensee to provide report copies to all parents/guardians with signature acknowledgement within 12 months.
