What Happened
The school-age program was otherwise operating in compliance on ratio, physical plant, food service, and outdoor safety. Deficiencies were cited for incomplete staff training and form issues, as well as medication storage and consent problems in children's files.
Observations
- Two staff members lacked a mandated reporter training certificate and two others only had the general training certificate.
- Two staff members needed Health Screening Reports.
- The substitute LIC 700 form was missing the child's gender field.
- The substitute LIC 627 form was missing a medication-allergy section and five children's files lacked that information.
- Two child medications were not stored in their original packaging with the prescription label.
