What Happened

Child breached outdoor playground fence and accessed area adjacent to street for 5-10 seconds before staff noticed. Facility immediately installed additional fencing to secure perimeter after incident.

Observations

  • Child slipped through opening in outdoor fence to unsupervised area adjacent to street
  • Child was outside licensed play area for 5-10 seconds before being discovered
  • Outdoor fence opening allowed child to access area not contained within licensed activity space
  • Facility immediately corrected issue by installing additional fencing
