What Happened

Case management inspection following self-reported incident where child exited classroom unsupervised for 3-5 minutes through enclosed hallway to gated outdoor playground. Staff had turned off classroom door alarm. Facility installed child-proof gate and blocked alarm switch to prevent recurrence.
