What Happened
Unannounced 3-year inspection found preschool program with generally good facility maintenance and food service practices. However, critical supervision violations observed: one child was left alone in the classroom while staff and other children were on the outdoor playground, and another child was observed alone in the bathroom without staff visual supervision. Both constitute violations of continuous supervision requirements.
Observations
- Child observed alone in classroom while all staff and other children were on outdoor playground—not under visual supervision (Title 5 regulation 809D violation)
- Child observed alone in bathroom without staff visual supervision (Title 5 regulation 809D violation)
