What the inspector wrote

On January 22, 2026, Licensing Program Analysts (LPAs) Kassandra Medrano and Diana Campos conducted a subsequent investigation regarding the above allegations. Present in the facility were 38 children, and 4 staff. It was alleged that there was an incident that resulted in a staff inappropriately handling a child while in care resulting in their hair being pulled. Based on the information gathered, there is not a preponderance of evidence to prove that staff inappropriately handled a child. Therefore, the above allegation is determined to be UNSUBSTANTIATED. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violation did or did not occur. A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the facility representative, Anjum.

  • PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights:(1) To be accorded dignity in his/her personal relationships with staff and other persons.this requirement was not met as evidenced by: Based on interviews and review of supportive documents it was found that there were instances where staff inappropriately handled and inappropriately spoke to children in care. This poses a potential health and safety risk to children in care.

  • PERSONAL RIGHTS

    (a) The licensee shall ensure that each child is accorded the following personal rights: (3)To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.This requirement was not as evidenced by: Based on interviews and documents recieved it was found that staff will tell children that they will take videos or photos of children and send to their parents if they do not listen. This poses a potential health and safety risk to children in care.
